SMS Program Workflow

1. Overview

RevHive Health LLC ("RevHive," "we," "us," or "our") is the SMS platform used by independent outpatient physical therapy clinics (each a "Clinic") to send patients account-balance notifications, payment reminders, and payment confirmations on the Clinic's behalf.

RevHive operates as a HIPAA Business Associate of each Clinic under a signed Business Associate Agreement (BAA). The Clinic remains the Covered Entity and the source of patient consent; RevHive provides the technology infrastructure that delivers compliant outreach under the Clinic's name.

Patients are opted in to the SMS program by their Clinic at intake. No SMS message is sent to any patient until the Clinic has confirmed that patient's written consent at the time the patient roster is uploaded to the RevHive platform.

2. How Opt-In Works

Opt-in to the RevHive SMS program is collected by the Clinic at the point of care. The end-to-end workflow is as follows:

  1. Step 1 — Visit: The patient visits the partner Clinic for physical therapy services.
  2. Step 2 — Intake paperwork: At intake, the patient completes the Clinic's new-patient paperwork. The paperwork includes a clearly-labeled SMS consent checkbox alongside the Clinic's Notice of Privacy Practices and other standard intake forms.
  3. Step 3 — Consent: The patient reviews and signs the consent language (full text quoted in Section 3 below). Consent is captured in writing on the Clinic's intake form and retained by the Clinic as part of the patient's record.
  4. Step 4 — Roster upload: The Clinic uploads the patient roster to the RevHive Health platform via our secure dashboard. For each patient included in the upload, the Clinic affirmatively confirms that consent has been obtained.
  5. Step 5 — Outreach: RevHive Health, on behalf of the Clinic, may then send SMS messages to the patient regarding the patient's account balance, in accordance with the Clinic's configuration, applicable send-window rules, and the patient's ongoing opt-in status.

3. Sample Opt-In Consent Language

The following is the verbatim consent language patients review and sign at the Clinic. The phrase [Clinic Name] is replaced with the legal name of the specific Clinic presenting the form.

“I consent to receive SMS text messages from [Clinic Name], sent on my behalf by RevHive Health (revhivehealth.com), regarding my account balance and payment reminders. Message frequency varies based on the status of my account, typically up to 4 messages per month. Message and data rates may apply. I understand I can opt out at any time by replying STOP to any message, and I can reply HELP for help. This consent is not required as a condition of receiving any healthcare service. 4. What Messages You'll Receive

Messages sent through the RevHive SMS program fall into three categories. The examples below use Demo PT as a placeholder Clinic name; in practice, the Clinic's actual name appears at the start of every message.

Balance notification

Demo PT: You have a balance of $45.00. Pay securely: revhivehealth.com/p/ab12 Reply STOP to opt out, HELP for help.

Payment confirmation

Demo PT: Payment of $45.00 received — thank you. View receipt: revhivehealth.com/r/ab12

Opt-in confirmation (sent after the first message or after replying START)

RevHive Health: You're subscribed to recurring account balance notifications. Msg frequency varies. Msg&data rates may apply. Reply HELP for help, STOP to cancel.

5. Keyword Commands

The RevHive SMS program honors the standard set of carrier-required keywords. Replies are case-insensitive and may be sent to any RevHive message.

Opt-out: STOP, UNSUBSCRIBE, QUIT, CANCEL, END

Replying any of these keywords immediately removes your number from the program. You will receive a single confirmation message and then no further messages unless you re-subscribe.

You are unsubscribed from [Clinic Name] account balance messages sent by RevHive Health. No further msgs will be sent. Reply START to re-subscribe.

Help: HELP

Replying HELP returns program identification and support contact information.

6. Supported Carriers

The RevHive SMS program is supported on AT&T, T-Mobile, Verizon, Sprint, U.S. Cellular, Boost, Cricket, MetroPCS, and other major and regional U.S. carriers.

Carriers are not liable for delayed or undelivered messages. Message and data rates may apply, as set by your mobile carrier.
